New claims may be submitted to claims@momentumhealth.co.za as an attachment and this should be in one of the following formats :
JPEG, JPG, JPE, GIF, PNG, PDF or TIFF.
Kindly ensure that your bank details are updated with us, should a refund be due to you. Should you wish to update or validate the banking details, kindly complete the changes to banking details form (and attach a copy of your ID)
