Below is a step- by-step guide on how to obtain your claims statement on the website:
- Visit www.momentum.co.za;
- Click on 'Log in' on the top right-hand side of the web-page, you will need to enter your user-name and password;
- Once you have successfully entered your user-name and password a new page will open;
- This should bring up a page entitled 'Portfolio view'; under the 'Health and Wellness' section your membership number should appear; click on your membership number;
- A new page should open with your entire contract details reflected; on the left-hand side of the page there will be a list of different options to choose from;
- Click on ‘Claims statement’, which can be found under the 'Claims' tab;
- A new page should open giving you the opportunity to select the statement month;
- Select the month and click on submit.
