Claims can be submitted by submitting your claim for a refund to health4merefunds@momentum.co.za. Please keep a reference number that you will receive on email to track your claim. Our turnaround time for refunds is 5-7 business days, provided that all documentation has been received. Please include the following documents for all refunds submitted to Health4Me so that they are processed quickly:
- ID copy of main member
- A detailed claim (with member number on it)
- Proof of payment (such as a receipt or bank statement)
- Bank statement or proof of banking details not older than 3 months Please note that benefit rules and limits still apply on refunds.
